Sample A21-A Printing Request Form

  1. AGENCY
    Enter the title of your organization.

  2. AGENCY NUMBER
    Enter your organization’s state identification number. This number is assigned by the Office of Financial Management for state agencies, and by DES Printing and Imaging for political subdivisions.

  3. DATE
    Enter the date the order is being submitted to DES Printing and Imaging.

  4. REQUISITION NUMBER
    Enter the requisition number authorized by your organization’s fiscal office. This number will be used to bill your organization for the cost of the order, so this area must be completed before we can proceed with your job.

  5. QUANTITY
    Enter the number of finished copies ordered.

  6. BOOKS, SETS, SHEETS
    Check the appropriate box to indicate what the quantity ordered refers to. Books include pamphlets and other work that will be bound. Sets indicate multiple sheets to be printed and collated. Sheets refers to single sheets of paper.

  7. JOB NAME OR FORM TITLE
    Enter a descriptive name for your job. You don’t need to include the name of your organization, but you may want to include a project or division name.

  8. FORM NUMBER
    Leave this area blank unless you are ordering a form. If you are ordering a form, indicate the specific form number, which is usually printed at the lower left or right corner.

  9. DATE REQUIRED
    Enter a specific month, day, and year. Do not use ASAP. Whenever possible, allow a reasonable period of time to complete your order, because shorter turnaround times can increase costs. The Frequently Asked Questions section contains guidelines for standard turnaround times. If you’re requesting a blueline proof, please include the amount of time you’ll need to review and return the proof in your turnaround time. If your order must be completed by a rush due date, please indicate that the date given is firm by writing 'must' or 'firm' beside the date.

  10. SIZE
    Enter the finished size of the printed piece. Enter the size with the width first, then the height. If you’re ordering bound books, the finished size entered should be the dimensions of the closed book.

  11. ENVELOPES, FORMS, PUBLICATIONS, OTHER
    Check one of these boxes only if it applies to your order. 'Publication' refers to jobs printed by state agencies intended for general dissemination to the public or legislature, such as annual reports, newsletters, and informational brochures. 'Other' refers to special printing of buttons, bags, signs, etc.

  12. NEW, REVISED, REPEAT
    Check the box which applies to your job. Mark 'Repeat' if the job is being reprinted with exactly the same text and artwork as the previous printing. If the job is a revision or repeat, please enter the previous requisition number and date of the last printing in the specified box (Number 21 on the sample A21-A).

  13. PAPER STOCK (TYPE & COLOR)
    Enter the weight, brand, and color of the paper needed for your order. When appropriate, please include your choice of cover stock. If you need more room to enter your paper selections, please write or type them in the 'Other Instructions' section. If you are unfamiliar with the variety of paper available, we can provide samples and assist you in your selection.

  14. INK COLORS
    Specify the exact ink color or colors to be used in printing your job. Please use both the Pantone number and the name of the ink. For example, 347-Green is the standard state color for green. If you don’t know the Pantone number of the ink color you want, our production planning staff will be happy to assist you. You can also purchase a Pantone ink color guide through DES Printing and Imaging for a nominal cost.

  15. AGENCY FURNISHES
    Mark the box or boxes that indicate what you will furnish to DES Printing and Imaging. Please tell us about any details or additional material, such as photographs, to be furnished in the 'Other Instructions' section of the A21-A.

  16. PROOF REQUIRED
    Check 'Yes' if you need to inspect a blueline proof before printing begins. Also provide the name, phone number, and mail stop of the person we should contact when the proof is available. If you would like a color proof or a press check, please include this request in the 'Other Instructions' section of the A21-A.

    Check 'No' if you do not wish to see a proof.

  17. BINDING
    Please type or print the binding method required for your job. Leave this box blank if no binding is needed for your order. If you have long or complex binding instructions, please enter them in the 'Other Instructions' section of the form or on a specifications sheet attached to your requisition. Please contact our Production Planning staff for advice on the best binding method for your job.

  18. PACKAGING
    Please indicate any special packaging requirements in this area, including shrink wrapping, bundling, and desired quantity per box. If this area is left blank, we’ll box your order in the most efficient manner possible, at the discretion of our shipping staff.

  19. ESTIMATE NUMBER
    If you requested an estimate before placing your order, record the estimate reference number in this space.

  20. CONTACT PERSON NAME AND PHONE NUMBER
    Enter the name and phone number of the person who should be contacted if we have any questions concerning the job.

  21. PREVIOUS REQUISITION &/OR PRINTER’S JOB NUMBER
    If this is a repeat job or a revision, enter your organization’s requisition number or the printer’s job number used on the previous order.

  22. DELIVER COMPLETED JOB TO
    Enter the address where you would like your printing order to be sent. Please include a street address, building name, building floor or wing, and room number. It’s also helpful to include the name and phone number of the person responsible for accepting the delivery. Please don’t use a mail stop number for delivery. We need as much information as possible for our drivers to deliver your order.

  23. REQUESTED BY
    Enter your first and last name, your job title, and phone number. Please sign where indicated. Note: If this requisition is for a publication as defined in RCW 40.07, the head of your state agency must also provide an approval signature. See the Appendix E to find the definition of a state publication.

  24. OTHER INSTRUCTIONS
    Use this space to give us any instructions or notes not included in other areas of the A21-A, in addition to specifications that won’t fit in the smaller boxes on the form. Please include the contact name and address where you’d like us to return the original artwork in this area as well.
